**Verifying Your Insurance Coverage for Nutrition Services Please note: It is each client’s responsibility to confirm their insurance benefits before scheduling an appointment. While we do our best to verify your coverage as a courtesy, any charges not covered by your plan will be your responsibility. How to Check Your Nutrition Benefits: Call the member services number on the back of your insurance card. Ask to speak with a representative and say: “Do I have benefits for nutritional counseling with a registered dietitian?” If they request procedure (CPT) codes, share the following: Primary codes: 97802 (initial session) and 97803 (follow-up session) If not covered: Ask if they allow 99401–99404, S9470, G0270, or G0271. They may also ask for a diagnosis (ICD-10) code. You can provide: Most common: Z71.3 (dietary counseling and surveillance) Alternative: Z72.4 (inadequate physical activity) If you’ve been diagnosed with any of the following, mention them as well: -Chronic kidney disease -Overweight or obesity -Pre-diabetes or diabetes -High blood pressure -High cholesterol -Obstructive sleep apnea -NASH (non-alcoholic steatohepatitis) or MASH (metabolic dysfunction-associated steatohepatitis) We bill using preventative care codes whenever appropriate to help maximize the number of visits your plan may cover. Make sure to verify that telehealth is covered by your plan. If mention a "place of service", ask them which is covered by your plan: 2, 10, or 11 with modifier 95. 💡 Please keep in mind: Certain diagnoses, such as IBS or other conditions without a qualifying diagnosis like obesity, may lead to out-of-pocket costs depending on your deductible, copay, or coinsurance. Your insurance company also determines how many visits are covered annually. Please note - NuSparkle Nutrition CAN bill as an out-of-network provider. The client pays upfront and an insurance claim will be sent to the insurance company for reimbursement. If the services are covered, the client is reimbursed the amount the insurance company paid. *You can also request that your health insurance company provide a coverage exception if they are unable to provide you with an in-network dietitian nutritionist. Your health insurance plan MUST be able to deliver the benefits you are paying for in your plan.
